What is a hazard? List four types of hazards you might find in the school environment and give an example of each.

A hazard is a situation that increases the odds that someone will have some sort of accident. One hazard might be ice on the front steps on a cold day. Another hazard could be a sharp edge on playground equipment. Another playground hazard would be inadequate cushioning material under playground equipment. Hazards in schools aren't always obvious, and another hazard might be a fire extinguisher that hasn't been serviced and would not work in an emergency.

List three factors you would consider when taking a group of children outside for ball games.

There's always some risk when children participate in sports, but a few steps can make injuries less likely. It might be smart to ask children to wear long pants so if they fall (or slide into base) their legs are protected. The children should also wear good shoes: sandals and flip-flops don't give their feet a secure footing. Also, before playing a game that uses something like a bat, they need to practice dropping the bat, not throwing it, so no one else will be hit by it.

3. Give four reasons why it is important to follow workplace procedures accurately.

Procedures are in place for a reason. We may not always know all the reasons, some include: so there is an accurate report of the incident, so what happened can be communicated accurately to parents, so if any disciplinary actions are required the person doing that knows exactly what happens, and to provide an accurate report in case someone threatens or takes l

4. Why are all accidents recorded and reported?

All accidents must be recorded and reported because the person in charge doesn't always know how serious an incident is at first and because questions may come up later about what happened.

We have fire drills once a month; we have a bell system for emergency lockdown (if the bell rings two short rings, each teacher pulls the blinds and locks the door); and the office has established and written procedures to follow in case someone is injured seriously or becomes suddenly very sick.

We have a Safety Committee. Any staff member, including janitors and cafeteria workers, can notify any member of the Safety Committee, and the safety committee will evaluate the concern and then present it to the school principal.

7. Write a paragraph on how employees can participate in workplace health and safety at your school and what your role could be as a Teachers' Aide

In our school, all instructional staff, including teacher aides, serve on various committees. They are more likely to spot playground safety problems than other staff members. Each year at the opening staff meetings, staff members volunteer for the committees they want to serve on. So all I would have to do is volunteer for that committee.

8. Identify a workplace occupational health and safety issue at your school. How would you report it appropriately? Make breif notes explaining the process.

If I saw a safety issue, for instance splintering wood on playground equipment, I would report it to someone on that committee. Someone on that committee would then go with me to the playground so I could show them the problem. They would take it promptly to the principal so a repair order could be put in. Since teacher aides do a lot of playground duty, we are more likely to 9. I attended one of these meetings. The people who attended were the members of the committee, three teachers and one teacher aide. The committee had three issues before them: the cafeteria workers noted that the gloves for handling hot pots were wearing out and they needed new ones; someone reported that lights were out on the parking lot, making the parking lot dark for nighttime activities, and someone noted that on rainy days, the front hall was very slippery because the front mats were not absorbing much water. The committee recommended that the school purchase new hot gloves for the cafeteria, have the maintenance staff check the parking lot lights, and get larger mats for the entranceway.

10.Make a copy of an accident report; make brief notes to explain how it would be completed. e.g. who fills it in, who is the designated personnel according to workplace procedures where does that person get the information etc.; the procedures for dealing with accidents, fires and emergencies.

The person who fills it in is either any adult who witnessed it (every adult who witnessed the incident fills out a form), or if no adult saw it, the adult in charge summarizes, as best he or she can, what they were able to find out about the incident at the time. The time and date have to be noted, along with any other pertinent information. If the accident happened outside, the weather may be relevant, for instance, if the child slipped and fell, and it was raining.

For emergencies, we have several procedures. In case of a fire, the directions are very specific. Every classroom has a map of the school mounted on the wall by the door. This map shows the route that class is to take out of the building. The route takes into consideration both what the shortest route would be and congestion: is it better to have six classes going out one door, or is it better for a couple of those classes to walk slightly farther to a different exit?

The Fire Department has helped the school determine where the staff and students should wait so they will be away from any fire as well as the fire fighting equipment. Each class has an assigned space on the playing fields, well away from the building.

Every teacher leads his or her students out, and takes the role book. As soon as they are out of the building, the teacher takes roll to make sure everyone out. If someone is missing, the teacher has a red card he or she holds in the air, and the principal or counselor comes to find out who the missing child is.

If children are out of the classroom, for instance for speech therapy, that specialist brings all the students out and returns them to their assigned class as quickly as possible so that all children are with their assigned teacher.

In the case of a lockdown, this would occur if someone in the school was afraid that someone was in school who might want to do harm to a person or persons. While someone coming in with a gun might be very rare, occasionally a non-custodial parent will try to get into the school and remove a child he or she has no right to remove. Since the office can't be certain where all children are at all times, they would issue a lockdown warning: two short rings. Each teacher draws the blinds so no one can see in, and locks the door. The children are instructed to be very quiet and allowed to read. Each classroom in our school has a phone, so if the office needs to communicate with a classroom, they use the phone.

11. Briefly explain how you would report any changes to an emergency evacuation map that you think are necessary.

It's very important that all the staff know where they are to take their children and that they don't just change that plan on their own, because where the closest exit is may not be the only consideration. If a teacher feels that the evacuation plan for his or class is not the best one, he or she simply takes it to the Safety Committee. The Safety Committee does not rule on these issues but gathers all the information it can on any issue brought before it, so the principal has as complete a picture as possible when considering the issue. In the case of fire evacuation plans, these plans have been worked out with the fire department, so the principal would confer with the fire department before making any changes.

12. List 3 considerations you would make when planning an "under eights" activity day to ensure effective and socially and culturally appropriate outcomes.

Such days should be planned by a committee that reflects the diversity of the students, including parents. Having parents participate is the first way to make sure the activities are culturally appropriate and not offensive.
