If these boundaries are violated, it can create a situation that can hinder the treatment of the illness or disorder. The biomedical model of disease also has some limitations in regard to many of its assumptions. For example, it assumes that disease to be fully accounted for by deviations from the norm of measurable biological variable (somatic) and leaves no room within its framework for the social, psychological, and behavioral dimensions of illness which is reductionist in form and assumes the language of chemistry and physics will ultimately explain the biological phenomena that is being observed (Caplan, McCartney, & Sisti, 2004). However, many theorists believe that this approach is too mechanistic and should include more of a systems approach which would include factors such as the general quality of life, clean environments, psychological and possibly spiritual stabilities in life (Hewa & Hetherington, 1995).

Furthermore, the composition of care under the biomedical model is continuously evolving. For example, the nursing staff is becoming more and more responsible for duties that would formerly only be conducted by doctors. This may include performing many different bedside caring duties during patient visits. This adds to the complexity of the professional duties that are performed by assistants and can add to the likelihood that they are often distracted during the jobs and thus can make professional boundaries even harder to maintain. There are estimated to be over a half a million health care assistance in the UK who now must all be professionally trained and qualified (unit 4). Discussion

When care work is carried out by a health care assistant it is generally under the supervision of a registered nurse. This can often create tension in the various positions that the patient is exposed to. For example, a patient may feel more comfortable with a health care assistant than a registered nurse due to the fact that they may see them more regularly. Therefore the patient may ask questions of the assistant that they may not be qualified to answer regarding their care or virtually any number of other topics.

For example, Anwar might have preferred to discuss his situation with the Health Care Assistant as she was the one providing the bedside care. It is a very real possibility that the patient felt more comfortable with the assistant because he rarely got to see the registered nurse in person. Furthermore, the health care assistant may have a natural desire to answer the patient's questions because they have also become comfortable with the patient. They may feel like they know the answer to his questions and wish to give their opinion in order to provide the patient comfort. They would naturally have the need to make him feel more 'cared for' and wish to address all of his concerns. This is an example of a scenario in which the assistant must be very careful in maintaining their professional boundaries. Despite the feeling of trying to help the patient, the assistant must be very careful not to go beyond their boundaries because they are not necessarily qualified to represent the position of the hospital and could ultimately jeopardize the patients care by offering information that is not accurate.

GUIDANCE In units 2 and 3 you read about Anwar Malik's encounters with a range of healthcare services and workers, and Ann and Angus's exp;eriencers when Yetund arrived on the scene. YHou considered much of it from the perspective of the service user , but you also observed the different ways care workers managed their practice to meet certain needs and demands. in particular, the somebody carers case study illustrates ideas about managing the relationship boundaries between workers and service users in practice.

For the first part of the question, you might want to describe briefly the different boundaries etween professional roles, as well as the service user care worker boundary, which you have been learning about in block 1, explaining why they are important. you can think quite broadly abound boundaries when considering their importance, but you need to make it clear in your essay which boundaries you are discussing. for example, in unit 2 section 4 you read about Anwar's experience of different roles within hospitals. in unit 3 section 1, you came across some of the limitations of yetunde's role.

For the second part of the question, you need to explain how care workers can maintain suitable relationships, and for that purpose you can draw on the somebody cares case study to consider some issues in more depth. unit 3 activity 7 on the DVD explores the boundaries between work and friendship as experienced by the workers and care recipients of the somebody carers agency.
