The objective of this project is to improve quality of care and hospital efficiency by encouraging the use of evidence-based practice. In particular, this research proposes that evidence-based care employed at the bedside will lead to a significant reduction of nursing-related errors, and subsequent reductions in unnecessary financial losses by the institution. In this way, the objective is specific, realistic, and measurable.

Solution Description

The proposed solution entails a comprehensive educational program applied in the work setting that will introduce nursing staff to evidence-based practice.

Describe the proposed solution.

The proposed solution will consist of an intensive leadership development program. Nurse managers will be assigned leadership roles as mentors. They will remain in this capacity for the duration of a year, during which outcomes will be measured. Nursing staff will be mentored on the use of academic databases, encouraged to patiently verify healthcare solutions and interventions against current literature, and also offered opportunities for professional advancement corresponding with improvement in evidence-based practice.

Research highlights the importance of leadership in promoting behavioral changes in the healthcare setting. This solution addresses the current weakness in implementing evidence-based practice by addressing one of the primary causes: lack of leadership.

The healthcare organization in question has a large enough staff to accommodate the proposed solution. Nurse managers are currently not trained to serve as mentors, and this program will be challenging for them at first. However, the mentoring program will create an atmosphere of camaraderie, and nursing staff will become increasingly empowered.

5. Discuss the way (s) in which the proposed solution is consistent with organization or community culture and resources.

The organizational culture at Mary Martha Hoffman Healthcare Center (MMH) in rural northern Mississippi has already begun to shift from a hierarchical structure to one that empowers its nursing staff. Although bureaucracy still plagues many decision-making processes, the proposed solution is consistent with current organizational goals and well within its resource capacity.

1. Describe the overall plan for implementing the proposed solution.

The first step is to call a staff meeting to introduce the proposed solution. The proposal will cover background information detailing why evidence-based practice is important. Focusing on the issue of washing hands, a presentation will reveal ways in which evidence-based practice is already being used everyday in bedside practice. Then nurses will relate their experiences using evidence to support their daily working decisions. Nurses will discuss their experience, or lack thereof, with using academic databases.

Next, nurse leaders will be trained as mentors. They will be offered a workshop to refresh their research skills. The nurse leaders will then show their mentors how to use the academic databases and offer opportunities for specific searching and exploration related to current patient issues.

Finally, the plan will go into action. Each day, nurse mentors will tabulate how many times nurses made specific bedside decisions, and which of those decisions warranted research on evidence-based practice. Digital tools will facilitate reporting. By the end of the year, a large database of information will have been compiled for data analysis.

Resources needed include access to professional development seminars, which may cost the institution up to $50,000. Other resources include the possible purchasing of new computer terminals for access to academic databases.

3. Described the methods for monitoring solution implementation.

Monitoring will be accomplished by the researcher and also by the nursing leaders who are participating in the study as mentors. The mentors will be responsible for tabulating daily instances of nursing-related decisions. Also, the computer software will be tracking login and search data. Using this data will reveal when nurses use evidence-based practice and which procedures or decisions are most likely to be researched in nursing literature.

Based on knowledge of organizational culture and change, this proposed solution helps create a culture of empowerment in the Mary Martha Hoffman Healthcare Center (MMH). A theory of planned change was used to show that improving nurse leadership can lead to direct and measurable results in terms of quality of care and efficiency of nursing practice.

The implementation plan is feasible because the methodology and the measures are simple and not time- or resource-consuming.

The outcome measure is valid because it is objectively measured. However, it would be difficult to measure the extent to which errors would have been avoided due to the increased use of evidence-based practice. For this reason, the researcher will also compile data on the hospital's track record of preventable errors in the area of infection due to improper hand washing. Instances of preventable infection will be measured for the year prior to implementing the proposed solution and then during the year the proposed solution was implemented. This way, the measure is more reliable and valid as well as sensitive to change and appropriate for use.

Evaluation Data Collection

The computers keep track of unique logins and search terms, allowing the researcher to know how many times the databases were accessed, and which topics were being researched. Nurse mentors will also be monitoring their nursing staff for instances in which a decision was made using evidence-based practice. Both the objective computer data and the more subjective nurse mentor evaluations offer a rich insight into how the proposed solution is creating direct and measurable behavioral change among nursing staff.

The resources needed include the technological tools of measuring unique logins by nurses. The nurse mentors need to use a database to record their observations of the nursing staff.
