nursing plan for a 96-year-old female who has several medical concerns and medications. The patient lives in a residential care facility and the nursing plan subscribes to the Jean Watson Theory of nursing. There were five sources used to complete this paper.The field of nursing has evolved from many years and today it is a highly respected and scientific profession in which its nurses can specialize or generalize their practice depending on their professional goals and desires. Many nurses subscribe to one or more nursing theories that help them develop the type of nurse that they wish to become. The Jean Watson theory of nursing has gained popularity across America and continues to provide incoming nurses with a blueprint plan for their careers. Watson's theory is based in the total care of the patient including humanistic, scientific and compassionate treatment of each patient during each encounter.
NURSING PLAN
The subject of this nursing plan is a 96-year-old female who is currently being cared for in Crosslands Rehabilitation Center.
The patient has a medical diagnosis of: Dementia, Hypothyroidism, GERD, cataract eye problems, Iron defiency anemia.
The nursing diagnosis from those who care for the patients includes: Distured sleep patterns, dementia, depression, social isolation.
Her current medications include:
Current Medications: Compazine 5 mg, (1/2-hour before meals) helps to not vomit.
Aleve 1 capsule twice a day for arthritis
Demadex 20 mg, 1/2 tablet daily (Diuretic)
Synthroid 75

Namenda 10 mg daily (dementia)
Pepcid 20 mg, twice a day (Barretts esophagus)
Lexapro 10 mg, daily, depression
MEDICAL CARE
The plan will include the administration of medications. Because the patient has iron defiecient anemia she will receive one iron tablet per day. It is important that the iron be given two hours apart from the synthroid therefore the synthroid will be given in the morning before breakfast. The iron will be given with the dinner meal so that it will not make the patient nauseous.
Meds will be given as follows. An hour before breakfast the patient will be given Synthroid, demadex, namenda and pepcid. The Lexapro and the Compazine will be administered a half hour later and in 30 minutes breakfast will be given to the patient.
Just after breakfast the Aleve will be administered.
The second dose of Pepcid will be given with the dinner meal at the same time the iron is given and the second Aleve will also be administered at that time.

The care of the patient will follow the Watson Theory of Nursing which believes that all aspects of care are administered at all times. From the time the nurse enters the room and makes contact with the patient the care should be geared to making the patient feel comfortable with their care.
All behaviors directed toward the patient will be caring behaviors which are defined as: " Behaviors evidenced by nurses in caring for patients. "
Caring behaviors to be provided to this patient in particular will include comforting, honesty, patience, responsibility, touch, using the patient's name and listening.
Because of the patient's dementia and inability to make informed decisions about her care the nurse will listen and provide comfort while following the orders of the patient's chart without changing it even by patient request. However, the patient will be treated with respect and dignity at all times during her care.
