Interview Question Preparation view has to be taken that nursing is one of the most satisfying careers that a person can get into. At the same time, there are a large number of nurses who are working in the country, and the total numbers are more than two million! The nature of the job in nursing is also changing continuously as the development of medicine is also changing the nature of the nursing job. It is also interesting to note that the efforts spent by a nurse are not in vain, as the compensation that they receive for nursing puts the nurses in the above average income bracket. As a nurse, one faces difficult situations in the course of their careers, and we are in the process of discussing some of them.

When you commence work as a Registered Nurse you will be faced with many decision making situations. Typically, you may be caring for four patients:

One patient's oral medications are due

Another requires an indwelling catheter for urinary retention

The third patient has a low blood oxygen saturation

One patient is verbally abusive towards others in the ward

Explain how you would prioritize the care of these patients and justify your prioritization

The actions of a nurse would have to depend upon the seriousness and urgency of the action that the nurse has to take. The question with regard to the verbally abusive patient is results due to a reaction from the depression that the patient has been having. Since there are several different types of patients in the ward, the assumption which is taken here is that this is not a maternal ward which contains adult pregnant women. It is known that they are most susceptible to abuse as it has been seen that one-third of the adolescent and almost one-fourth of adult women have reported being abused during pregnancy. This sort of abuse leads to poor obstetric history, substance use and low birth weight. (Curry; Perrin; Wall, 1998) Keeping that in view, it seems that the abusive patient can be ignored till the time that the other patients are attended to. In short that patient will be number four in terms of handling. This will lead to more abuse from the patient, but that cannot be avoided.

The next patient who can wait is the one who requires an indwelling catheter. Just a word to the patient that the patient must hold on as you have to attend to another patient who has to be attended to first, may help the patient actually. It has been seen that in the case of older women facing incontinence problems could be reduced by 80.7% through behavioral treatment and that was more efficient in comparison to the treatment with that of drugs. It was also compared with placebo control condition on the same target group. In the perception of the patients also, it was viewed that behavioral treatment was far better as the number of persons satisfied with the treatment was the highest among all the three groups. Only 14% of the patients being treated through behavioral treatment wanted to change to another form of treatment as compared to more than 75% in the other groups. (Burgio; Locher; Goode; Michael; McDowell; Dombrowski; Candib, 1998) The process also involves a little time.

The third patient who is waiting is the person whose oral medications are being due. The timing for oral medication is generally fixed on an interval of four hours, and a few minutes during the course of those four hours should not cause irreparable harm or damage to the patient. The patient who has needs be attended on at first is the person who has low oxygen blood saturation. In this case we need to understand that if the patient is not being attended to, he may collapse. The low oxygen blood saturation means that the patient concerned will have to be given extra oxygen in the air that he is breathing. If there are some instructions from the doctor which are to be followed for this eventuality, then it may be followed, or the information may be sought from the doctor, or the patient concerned may be provided a burst of oxygen enriched air, till the situation when the oxygen blood saturation improves. After this person is considered to be relatively stable, or has been given enough access to oxygen enriched air, then the next patient who requires oral medication may be attended to, and this may be followed by the patient with the incontinence problem and finally the verbally abusive patient may be attended to.

As a Registered Nurse, what do you believe to be the skills and attributes required to practice effectively?

The first question that one has to think about is in relation to the entire responsibility of nurses and that is with regard to help the patients who come for treatment. Often enough the patients are not in a position to help themselves and their family members are either not available or are not able to help them. The family members or their friends usually come and visit the patients only for some hours in the evening, but during the rest of the day, it is the nurse who has the responsibility to look after the patient. The first responsibility of the nurses is thus to deal with a lot of empathy. The nurse should imagine that if the nurse is placed in such a situation which the patient is now involved in what type of help the patient may require. There are now several hospitals where the nurse in charge and they take a round in the midnight. This helps them have a clear idea/picture of the situation of every patient then. Some of the patients would like to talk and others are who do not like to talk. The situations of the patients who like to talk become clear when they mention it themselves, but for the others, the nurse has to find it out through means of observation. During the midnight visits the hospital person may be able to suggest changes of medication so that the patient may feel better. This might be a change in pain medication or reduce the frequency of a drug. (Transforming Care at the Bedside Spurs Innovation)

These help the patients get better faster. At the same time, the nurses involuntarily get dragged into events that are not really theirs. When there are questions of life or death for a patient, or conflicts within the family of a patient, the nurse has to become a witness. Sometimes, they even have to take steps so that the patient does not suffer from these events. Some patients are constantly demanding that they have new facilities or are complaining with regard to the new difficulties, and it becomes a great responsibility for the nurse to satisfy the patients in such a manner that the patients will be happy. However this is not such an easy job, as the nurse is also bound by the instructions which are available from the doctor. Most nurses are found to face these difficulties and the capable nurses are able to deal with such problems on their own. The second problem is of knowledge, and nurses are a part of a science that is growing very fast. Methods which were being used twenty years ago are no longer in use, and a capable nurse has to learn the new techniques so that the nurse can offer the patients the best that is available. It may be seen that the nurses of today are viewed as a part of a great body of knowledge, more accountable and more independent in their action. This is the view of nurses who had begun their careers some time back. At the same time, different social situations bring more problems to the nurses, and an example of this is that when people do not have insurance and still need care, they use the emergency department as a primary care unit. (Spotlight on emergency Department Nursing) This is happening even though the nurses in the emergency section would like to view it as being an area for immediate treatment of life threatening conditions of patients. This does not matter to nurses as the nurse must provide care for everybody who requires care. It has always been viewed as being a part of mercy. This is also the spirit that is given to the activity by social organizations and churches of all denominations.
