[EXCERPT] . . . .

In this regard, it is understandable that the medical and healthcare systems charge with implementing the new policy would be invested in patient education about the use of emergency rooms and willing to dedicate some resources to ensuring that this pattern changed radically as new enrollees moved into established healthcare systems and medical institutions.

It is important to recognize that when entire swaths of the national population are deprived of medical insurance, the nature of treatment and care will be impacted. According to the Insurance Commissioner of Kansas, Sandy Praeger, "In most cases, adults with incomes from 32% to 100% of the poverty level ($6,250 to $19,530 for a family of three) will have no assistance." Nurses are likely to experience this issue in their day-to-day encounters with patients. When basic care is not available to people, they enter the healthcare system with more acute or chronic conditions that require more aggressive and longer-term care than when medical issues are addressed earlier. In every way that a healthcare system can fail, the nursing profession is can be impacted. Alternately, when a healthcare system serves its members well, then more emphasis and resources can be directed toward preventative care -- with better outcomes for patients and more satisfying engagement for nurses.

The PPACA of 2010 fostered new provisions for health care and the structure of health care delivery. The individual mandate to obtain insurance is one provocative provision. While this provision attempts to increase access to health care, it raises questions on how the existing system could sustain the potentially large influx of newly insured individuals.

Another provision calls for new models of health care provider organizations to ensure delivery efficiency and continuity of care. In this week's media presentation, Dr. Kathleen White discusses the accountable care organization, which comprises a group of providers coordinating care across a variety of institutional settings. Yet becoming an accountable care organization may present a number of challenges.

This week's Discussion builds on Week 1, continuing the examination of those societal and organizational contexts that influence health care reform. The unintended consequences of reform policy on the health care system are also considered.

To prepare:

• Review this week's media presentation and the other Learning Resources focusing on how reform may lead to improved quality, greater access, and reduced cost of care. Also think about the unintended consequences that may arise as a result.

• Consider the information presented about the individual mandate and accountable care organizations. What are some questions or concerns you might have regarding the individual mandate? What are the pros and cons associated with becoming an accountable care organization? READ MORE
