I would administer two tests, one which would test her for drug and alcohol usage, and the other which would assess her eating habits. These tests are simply formalities, it appears to be certain that she suffers from these types of disorders; the tests would be given simply to determine the details and the extent of her disorders.

Treatment Plan

The treatment plan for this condition will truly have to be rigorous, seeing as how she suffers from several (though interrelated) conditions. For instance, "psychologists say that eating disorders, like other addictions, are often rooted in the need to numb emotional pain with substances or the rush provided by bingeing and purging. The disorders are often driven by childhood trauma like sexual abuse, neglect and other sources of mental anguish" (Kershaw, 2008). Getting to the root of Alyssa's emotional pain and mental trauma is going to take some time but these are generally the wounds and disruption which are fueling her abuse.

I would prescribe Alyssa outpatient treatment three to four times per week. While the treatment plan would be outpatient, I would issue a plan that was extremely rigorous. For example, the outpatient plan would consist of sessions Mondays, Wednesdays and Fridays. In the morning she would have one on one counseling for her eating disorder for an hour and a half and then she would have group therapy with other eating disordered patients for another hour and a half. Then she would be given a three hour break. In the afternoon she would have one on one substance abuse counseling and then group therapy with other substance abusers. her inter-related disorders.

However, there will need to be a treatment plan in place to help get the Alyssa through the first few days of withdrawal. It will be wise to admit her to a hospital or rehab facility which has a detox unit and which will have medical professionals who will be able to guide her through this process, check her vital signs, and give her medication, if absolutely necessary, to help her control her symptoms (Harris, 2012). Finding a treatment center which is covered by her insurance is absolutely necessary given the fact that this type of rehabilitation is extremely expensive; expecting Alyssa to go at it alone with only the support of her family to help her is most likely setting her up for failure. It's important to consider that Alyssa appears to be likely be the adult child of an alcoholic father: "Children of alcoholics are at the highest risk of developing alcoholism and are prone to learning disabilities, eating disorders, stress related medical problems, and compulsive achieving. Children of alcoholics also develop an inability to trust, an extreme need to control, an excessive sense of responsibility, and denial of feelings; these problems persist through adulthood" (Schultz & Videbeck, 2009, p.50). Many of the symptoms and issues that Alyssa is experiencing have to do with the fact that she grew up with alcoholism, she may have grown up with physical abuse or witness violence (the "cappolito temper") and she manifesting a range of conditions which can almost be viewed as post-traumatic stress symptoms. Basically, part of her treatment will have to involve building up her sense of self-esteem, insight, and encouraging her to develop coping and problem-solving skills (Schultz & Videbeck, 2009). This work will be done in conjunction with the specialized counseling that Alyssa receives.

Case Conceptualization Presentation

The purpose of this is to learn how to gather information about a client in order to diagnose the issue to be addressed, a direction for treatment, a rationale for the treatment and how to use forma assessment instruments to assist in diagnosing the presenting issue.

A case scenario is given after the instructions below. Using the information given from the scenario, you will be required to use the list on page 174-175 (which will be provided) to develop a profile of the client and using as much of the assessment in Appendix K (on page 449) that is appropriate to provide evidence of the diagnosis. You will need to be creative because you will not be working with a real client but a fictional client (on paper) in which you will be creating the answers to the questions. It is imperative that you keep your client believable. Try to keep in mind a real person. The paper and PowerPoint presentation will consist of you providing some demographic information about your client (make it up), the precipitating event which led to the current issue, your diagnosis of the primary issue, provide evidence to support your diagnosis from the clients past and current experiences, behaviors, etc. Also share any assessments you would use to further support your diagnosis and a rationale for using those assessments. The paper and PowerPoint presentation should conclude with how you plan to approach treatment of the client (i.e. treatment goals and objectives, modality of treatment, and rationale for the approach you have chosen). Modality of treatment refers to individual or group therapy, outpatient or inpatient, and/or didactic or process *****.

For the PowerPoint presentation create a profile of the client, the presenting issue and your ability to assess/diagnosis the primary issue, how you came up with the diagnosis, develop an effective plan to address the client*****s problem, and how and why you chose the treatment to address the client*****s issue. You will need to be able to demonstrate how you would use formal assessments tools as well as informal assessment methods to diagnose the client.

Case Scenario:

You are the counselor in an outpatient clinic. You will meet this client for the first time. This is 25-year-old single, Caucasian female. She has never been married. She lives with her parents. She recently lost a job. She has been very indecisive as to what to do next. She has no plans to find another job. She is sitting at home and has been obsessing about her life and all her mistakes. She is a recluse and is not maintaining her hygiene. She has not been eating properly and has lost weight since the job loss. The client does not believe there is anything wrong with her and that she didn't need to see you as a counselor. She believes everything is fine.

Diagnosis: clarity of the diagnosis of the client*****s issue. Treatment Plan: thoroughness of the treatment plan and its ability to effectively address the client*****s issue. The modality of treatment will need to be clearly identified with support as for using a particular modality. The treatment goal(s) and objectives must be presented for the primary issue. Provide some information on how treatment will proceed, for example what are some techniques that will be used to meet the goals and objectives of treatment.
