[EXCERPT] . . . .
Because of her age and the fact that the hole was in its early stages, only monitoring was first prescribed. A cautious approach was determined given surgery contains some risks, including an increased risk of cataracts. Infection and retinal detachment can also occur (Macular hole, 2014, NIH).After a three-month check-up a macular hole in the left eye was discovered as well. Surgery was immediately prescribed because of concerns of a loss of vision in both eyes: first, surgery on the right eye was performed, followed by surgery on the left eye. It should be noted that dual macular holes are not unprecedented but they are not common: on average there is a 10-15% patients will experience a macular hole in their other eye over the course of their lifetime (Macular hole, 2014, NIH).
Surgery (vitrectomy) involves removing the vitreous substance and replacing it with a bubble (Macular hole, 2014, NIH). After surgery, the patient must sleep face downward to ensure a successful result. Normally, this is necessary for a day or two but can be required for as many as two to three weeks. Eventually, the bubble is reabsorbed and new vitreous gel naturally fills the space (Macular hole, 2014, NIH). Air travel is prohibited for all patients for several months given that "changes in air pressure may cause the bubble in the eye to expand, increasing pressure inside the eye" (Macular hole, 2014, NIH).
Even post-surgery, D.Y. had poor vision acuity: before sugary her right eye was 20/200 and left eye was 20/70. The surgery did cause the blurriness to subside. Refraction was tested using eye drops and refraction era was zero. There was also improvement in eye focusing an

However, the cause of the hole is still unknown and six months later another macular hole was found. Despite this, it should be noted that patients who have the surgery six months after diagnosis have a markedly better recovery rate (Macular hole, 2014, NIH). Recovery was slow, which is normal regardless of the patient's age. When the gas bubble is still in place, the patients' vision is blurry and the sense of balance is affected. Within 7-10 days as the bubble shrinks, symptoms should subside although full healing takes 6-8 weeks for full absorption of the bubble (Macular hole, 2013, NHS).
Discussion
D.Y's case was unusual because of her age and the fact that both eyes were affected at the same time as well as the fact her case was not successfully resolved. It is a credit to her physician that such an unusual case was diagnosed correctly and taken seriously. Her case also underlines the need for expedited treatment of this condition which is usually highly treatable (90% of all surgeries within less than a year of treatment are successful) but is ideally diagnosed in the early stages (Macular hole, 2013, NHS).

Quoted Instructions for "Macular Hole" Assignment:
Hello,
Please follow below instruction, there are what my professor is looking for to finish this assignment and also include my proposal of this assignment. I will send out one example is provided by professor and Could you follow the same format as the example.
Here is my proposal
I am planning to interview my friend Divine Yang. She had Macular Hole on her both eyes. Macular hole is a small break that effect central vision retina. Some macular holes can seal themselves and require no treatment, but in majority cases surgery is necessary help improve vision. At the begin, the subject noticed that her vision was getting worse. In addition, she had episodes of blurriness in her right eye and vertigo with long time reading or using computers. The subject went to physician after two months after she realized these symptoms came out frequently. The physician listened to her describe her symptoms in detail and examine her. After differential diagnosis, the physician told her that her right eye was in early stage of a macular hole and left eye was in good conditions. Therefore, the physician decided not to use any treatments but instead the subject need to keep regular check-up every month. After three months regular check-up, the physician discovered her left eye had a macular hole as well. The physician suggested her does the surgery for her right eye as soon as possible and then three weeks later the subject finished both eyes surgery. My cousin had a macular hole on both his eyes as well.
Here is instruction:
Topic: Your task for the first paper is to write a case report modeled after read the formal case reports
The heart and soul of the case study is careful observation of a patient*****s history and behavior. Ultimately, whatever its form the case study presents a narrative, one that attempts to record and make sense of any meaningful physiological, biochemical, behavioral, and emotional characteristics.
As you are not yet health professionals, you may think that you don*****t have access to actual patients. However, you do already know a great deal about a variety of illnesses, diseases and disabilities, even if they*****re only common ones like the cold, influenza or childhood illness. Any of these topics might, with careful observation, provide the basis of the case study. So, too, might your knowledge of a more unusual or serious problem.
For this assignment, you must interview someone who has some kind of problem with their health and then write a report of the case.
Use the examples as models. Note that the style of these tends to be direct, professional and (apparently) objective. The sentences tend to be fairly short, and the reports don*****t offer a great deal of explicit or implicit analysis. Note as well that, because you are not yet medical researchers, you cannot write a full Discussion section. Your Discussion section will be brief, no more than a few sentences long.
Topic Proposal:
Before you begin, you must submit a paragraph that explains the subject of your proposed study. Who do you plan to interview? About what medical issue? What do you know already about this condition? About this person*****s experience? Who else will you be able to talk to about their experience?
Your case study must include the following:
The patient*****s age, gender, ethnic and socioeconomic background
Careful descriptions of the manifestations of the patient*****s health problem. These descriptions must be based on your own observation of the patient, his or her friends/family and the interviews you conduct.
Careful observation and description of pertinent behaviors
A discussion of the onset of the problem and its course. Some of this information may come from the interview; some will come from observation. You will want to consider possible triggers to the problem, the severity of episodes, the usual duration, the extent to which physiological and emotional responses change throughout the course of the problem and the frequency of the problem.
Responses to treatments.
Additional Requirements and Restrictions:
You may not use yourself as the subject of the paper.
You may not write about a patient you have met while working or volunteering at a hospital, emergency room or clinic.
Do not use the person*****s real name in the paper. Refer to the person either as *****the patient,***** or invent a pseudonym for them.
You must get your subject*****s written permission to write about him/her, and let him/her know the nature of the assignment. You must attach to the paper a signed, dated consent form from the subject. It should indicate that you have given the subject a chance to read your case studies.
Submit with your final paper, organized in a paper folder with pockets:
Detailed observation and interview notes. Your notes will necessarily contain more information than your paper does, as you won*****t know what*****s important or relevant until you begin writing the paper, and you may have to take several rounds of notes. If you submit notes that are insufficient or that appear to have been hastily written after the paper was completed, you will have to rewrite the entire assignment with a new topic.
A clearly marked final draft.
Any and all earlier drafts, including those that your peers mark on in workshop
Any other materials that you generate in preparing the draft (outlines, free-writing, etc)
The Writing Process and Time Management:
The process of writing the paper will certainly involve the following steps (and will probably involve more). In order to manage the work, you might try to establish deadlines for each of these tasks:
Task Finish by
Contact potential subjects _______
Submit topic paragraph _______
Conduct initial interview _______
Initial free-writing, idea generation _______
