Healthcare workers must ensure that the hospital protects vital health information such as names, dates, social security numbers, and other identifying information. These cannot be shared with third parties without the patient's consent. One of the primary purposes of passing HIPAA was so that insurance companies or outside parties with a vested interest in discriminating against patients with certain conditions could not have access to medical records that could now be easily shared with a click of a mouse in the age of electronic record-keeping.

Under HIPAA, patients also have the right to certain information: the "right to receive a notice of the privacy practices of any health care provider health clearing house, or health plan;" to correct errors in records, to have access to personal information, see a list of disclosures that have been made and to demand special treatment or confidentiality regarding their personal health data (What is HIPP, 2013, Department of Health). The presumption is to err on the side of protecting patient privacy. Particularly when dealing with patients who are not capable of giving consent, such as children or patients with dementia or mental illnesses, the question of who is authorized to have access to their medical information must remain clear. Healthcare providers cannot simply make assumptions.

It is "the expectation that one may be called on to justify one's action, beliefs, and/or feelings to others. Accountability typically triggers rewards for favorable accounts and/or sanctions for unfavorable accounts" (Rivard 2003). In short, it ensures that there will be consequences for specific actions, both positive and negative, and someone who performs those actions believes that standards will be upheld.

Reliable means that there is an assurance that a person, procedure, or form of technology will have a predictable result (preferably a positive one). A reliable source of care is trustworthy and does not require continual monitoring to ensure that it will perform its task. For example, a student nurse might not be considered 'reliable' because her grasp of professional procedures is still a work in progress, versus a nurse with a long record of service who had a proven track record in dispensing the needed care and treatment to patients. Treatments themselves also must be reliable and proven to work according to the accepted standards of the medical community. For a treatment to be prescribed it must have some reliable pattern of showing improvement vs. existing alternatives or non-intervention.

Diligent means observing the protocols demanded of the healthcare environment -- for example, in exercising due diligence to perform the necessary tests; dispense the necessary care and medications; and to record data in a manner keeping with standard operating procedures. Diligence means not 'letting things slide' and 'skipping steps' even during routine procedures. While no healthcare provider can be assumed to be 'perfect,' all can be diligent in terms of following accepted standards of evidence-based medicine and taking precautions to prevent errors and known worst-case scenarios.
